Mentoring/ Coaching Contract

This agreement covers a formal Mentoring/ Coaching relationship between ______________________ (Client) and Helen Dale (Mentor/Coach) for simplicity referred to hereafter as the Mentor.

The objective of this relationship is for the Client to gain guidance and support from the Mentor.

The Service

The services will consist of meetings and/or phone calls and/or email or other written communications between the Mentor and the Client from time to time, the exact content, frequency and duration of which will be as agreed between the Mentor and the Client.

The Mentor shall not at any time be obliged to provide or continue to provide any mentoring services to the Client.

The Client shall not at any time be obliged to request or receive mentoring services from the Mentor.

Gender Recognition Act Disclosure of Protected Information

Section 22 of the Gender Recognition Act 2004 (GRA) prohibits the disclosure of “protected information” except under specified circumstances.  Protected information relates to an application for a Gender Recognition Certificate or, if the individual has a GRC, to information about their previous gender.

This could prevent the Mentor from referring the Client to other services that might be appropriate including referring the Client to any support. The Client gives permission for the Mentor to disclose “protected information” where necessary to obtain support for the Client from other agencies and services. This permission to remain in force until the Client gives notice in writing withdrawing that permission.

Where “protected information” is passed on it will be to named individuals who have been advised of their obligations under the GRA not to disclose it further without permission from the Client.

Client Records

The Mentor may maintain case notes on the Client. These will not include details of the Client's name or address. These records may be kept on computer or as written or printed copies. The Client expressly consents to the maintenance of such records. Anonymised information may be used for training and inclusion in any books by the Mentor/Coach.


Sessions will be arranged as mutually agreed to meet the Client’s needs

The Mentor will make every effort to arrive on time.

Each session will last 50 minutes from the appointment time. If the Client is late, this will reduce the time available. The Mentor will wait 15 minutes if the Client is late.


At least 24 hours’ notice must be given of cancellation if possible.

In emergencies, the Client should attempt to notify the Mentor as soon as possible when it is clear that s/he is unable to attend. If s/he is unable to contact the Mentor/Coach, a message should be left on or a text sent to the Mentor’s mobile. The Mentor undertakes to reciprocate.


The fee shall be £       per standard session.    

Failure to attend without notice

If a Client does not attend a session and does not provide at least 24 hours’ notice of cancellation on two occasions, the Mentor will require a deposit of £40 against future “failures to attend without 24 hours’ notice” before further sessions can be arranged. In the event of any further “failures to attend without notice”, the deposit shall be forfeit and a further deposit shall be due before further sessions can be arranged.
